You have many options to schedule your appointments with us- you may call to schedule an appointment during office hours, schedule online or use ZOC DOC. Please contact us as soon as possible if you need to cancel or change your appointments.
Please state the reason for the Visit and provide the Insurance details before you make an appointment, this way we can verify your insurance before the appointment date and expedite your visit on the day of the appointment.
New Patients office visit, please plan to arrive in the office 15 minutes prior to your scheduled appointment so that you have time to fill out the paper works or update any new information. Please also bring your insurance card, drivers license or a valid picture identification card and co-payment if required. Please also bring a list of your current medications. We like to keep your appointment as short as possible because we know that your time is just as valuable. However, emergency does arise during the office hours. We will try to inform you of the delay as expeditiously as possible.
Patient Calls during office hours, your telephone calls will be answered as promptly as possible. Our staff members have been trained to answer many of your questions which allow the physician to attend to the scheduled patients with minimal interruption.
If there is an emergency or if you are in labor after office hours, please call 678 335 9223. The answering service will receive the call and triage the call accordingly, please provide your name and call back number, the nature of your emergency. They will get in touch with the doctor or the on-call physician.
If this is an Emergency, please hang up and dail 911 or you are unable to get in touch with us, please go directly to emergency room at Emory Johns Creek, NorthSide Forsyth, and North Fulton Hospital.
If you have any questions regarding your medical care, please call during our office hours. If the nurse is not available, you may be asked to leave a message and our nurse will return your call as soon as possible, If there is an emergency or if you are in labor, please ask our office staff to return your call right away.
Prescription refills are processed during office hours only. Please provide us with a telephone number of a major pharmacy (CVS, Walgreen, HEB, Wal-Mart ) close to your home or work. If you have not been seen for more than six months to a year, please schedule an appointment with our office before your prescription can be refilled.
